Tax Relief Application Form Completion Notes for Civil Servants
If you are a government worker, please note the following when filling out the tax relief application form each year for the smooth processing of your applications.
Year
Write down the year you are applying for relief.
It should be noted that the year cannot be in the past. For example, you cannot apply for tax reliefs for 2023 in 2024.
Employee Surname
Write your surname as it appears on your payslip.
Other Names
Write your first and other names as they appear on your payslip in this section.
Gender
Tick the one which applies to you.
Date of Birth
Write your date of birth in the format Date: Month: Year.
For example, 13/07/1999 for 13th July 1999.
Mother’s Maiden Name
Write the full name given to your mother before marriage.
Social Sec. No
Write your social security number here.
Tax File No.
Write your Staff ID Number in this section.
Please make sure a valid Staff ID is put here since the system will reject any application with an incorrect ID or when there is a name mismatch with the ID provided.
Name of Employer
The name of your employer should be written here. For example, Ghana Education Service for Teachers, Ghana Health Service for Nurses, etc.
Employers Address
Write the name of your current station here.
Telephone Number
Write your mobile phone number in this section.
Tax ID No.
Write your Ghana Card Number here.
Marital Status
Tick the one that applies to you. There are only two options in this section, Married or Single. Please choose between the two options provided.
If Married Name of Dependent Spouse
Provide your spouse’s full name in this section
Particulars of Spouse
a) Date of Birth
Write the date of birth of your spouse here.
b)Tax ID No.
Write your spouse’s Staff ID number here if he or she is a civil servant.
c) Social Sec. No
Write your spouse’s social security number in this section if applicable.
d)Tax File No
Write your spouse’s Ghana card number here.
No. of Children
Please indicate the number of children you are applying for. The maximum number of children is 3.
If you have more than 3 children, the best you can do is get your spouse to apply for the rest since two parents cannot claim the relief for the same child.
Particulars of Children
Indicate the name (s) of the children (up to 3), their date of birth and the name(s) of the schools they attend in the respective columns in this section.
Are You Disabled?
Tick the box that applies to you. If you tick Yes, you must attach a disability certificate or letter from the Department of Social Welfare.
Declaration of EMPLOYER
Get your Head or Supervisor to sign in the signature column of this section. He or she should write his/her full name in the declaration space and write the date he or she signed the form.
If he or she has an official stamp, he or she should stamp the form in this section as well.
Declaration by EMPLOYEE
Make sure you sign and date the form in this section after ticking Yes or No to the question about having other sources of income apart from employment.
Aged Dependents
Remember to write the names, date of birth or age, and relationship of up to 2 Aged dependents at the bottom or the back of the form.
The names could be the details of your Mother, Father, Uncle, Aunt, etc, who must be 60 years and above at the time of your application. You cannot use siblings for the aged dependent relief.
